BME HEALTH AND SOCIAL CARE SUPPORT SERVICES
The aim of the project is to provide support services to elderly BME (Black Minority Ethnic) Women and Men living in
Cardiff, to help them lead Independent lives and to be able to access health and social care support services.
The Project Objectives

Health Awareness Sessions – Focus on Health issues such as Diabetes, Blood Pressure, Heart Disease, Stroke, smoking cessation, falls prevention.
The aim will be to encourage self care management and to encourage BME groups to access services which will help them manage these conditions.

Cultural Awareness Sessions – Cultural Awareness Sessions will be organised for service providers to raise cultural issues which impact on access to services especially in relation to older people accessing mainstream health care.

Hospital Discharge Support Services – Trained volunteers will help with hospital discharge by accompanying them home, assessing immediate needs, assisting with low / moderate support needs such as shopping and preparing meals.

Health Information Leaflets – Information leaflets on accessing health services will be produced in various Ethnic Minority languages.
